Estimate rights
You have the right to receive a Good Faith Estimate explaining how much your health care will cost. If you are uninsured or are not using certain types of health coverage for your care, federal law generally requires health care providers and facilities to give you a written estimate of expected charges. You can request an estimate for the total expected cost of health care items and services, including related costs such as medical tests, prescription drugs, equipment and hospital fees.
When you should receive an estimate
If you schedule care 3 to 9 business days in advance, you should receive the estimate within 1 business day after scheduling. If you schedule care 10 or more business days in advance, you should receive the estimate within 3 business days after scheduling. You can ask for an estimate before scheduling; you should receive it within 3 business days after your request. Scheduling care fewer than 3 business days in advance does not by itself require an estimate before that visit. You can still request one. Ask for a paper or electronic copy in your preferred format, and keep it with your bills.
Practice fees and outside charges
The provider fee is $75 for a standard encounter, or $150 total when additional diagnostics or labs need to be ordered or reviewed. These are alternative encounter fees, not amounts added together. Separate facility, laboratory and imaging charges are not included in the provider fee. Your individualized estimate will identify the expected practice charges and any separately scheduled services. Separate billing does not waive your rights or change a provider’s legal responsibilities.
If your bill is higher
If the bill from any provider or facility is at least $400 more than that provider’s or facility’s Good Faith Estimate, you may be eligible for the federal patient-provider dispute resolution process. Start promptly and within 120 calendar days after receiving the initial bill.
